How To Make A Referral
Admitting a patient to hospice is a simple, time-saving process for the
attending physician, discharge planner or social worker.
The following steps apply:
- Call the hospice office with the patient's name, address, age, present
location, diagnosis, and estimated prognosis. (Often this information is sufficient to
make an admission decision.) 912-272-8333 or 1-866-475-5233.
- A hospice representative will visit the patient and family at home or in
the hospital to assess the immediate needs, recommend an initial plan of care and obtain
an informed consent.
- Care will begin within 1 working day.
Referrals Accepted From:
- physicians
- hospital personnel*
- family and friends*
*Requires confirmation from primary physician as to appropriateness of
hospice care.
*Patients are admitted to Hospice and are provided services without
regard to any form of discrimination.
